To achieve good health, you have to take advantage of every support tool available, including the confidential health resources MPTN offers at no cost to eligible Team Members: a Wellness Incentive, reduced-cost gym memberships and personal wellness coaches through the Disease Management Program. Participation in the Wellness Program is voluntary.
Wellness Incentive
Start now to qualify for your 2023 Wellness Incentive. You and your spouse (if covered by an MPTN Medical Plan) must complete the following simple, no cost steps by September 30, 2023. Any steps you’ve taken since October 1, 2022 will count toward your incentive.
Step 1: Get a Check up
Annual physicals are 100% covered when you visit an In-Network provider. Remember to speak with your doctor about coding your annual physical appropriately to avoid a $25 copay. Visit www.myezhealthguide.com for details. To locate a network provider, visit www.welcometouhc.com.
Step 2: Know your Numbers
The biometric screening can be completed by your doctor or through Pequot Plus Health Benefit Services.
Step 3: Manage your Health
If you were diagnosed with a chronic condition prior to October 1, 2021, the last step toward earning your incentive is to manage your health by meeting minimum care recommendations for your condition. Keep in mind, not everyone will have to complete this requirement.

You’ll need to perform all of the services that are recommended for your condition during the tracking period, October 1, 2022 to September 30, 2023.
$$$ You will automatically be reimbursed by Pequot Plus Health Benefit Services for the cost of any In Network office visit copays related to managing your chronic condition. Additional services will be subject to deductible and coinsurance.
Disease Management
For Team Members living with a chronic condition, it’s never too late to take control of your health. That’s why the Disease Management Program, now offered by the American Health Data Institute (AHDI), is so important. It’s the support you need to understand your condition and develop a personalized treatment plan.
If you or a family member (covered by an MPTN Medical Plan) are diagnosed with a chronic condition, you will automatically be enrolled in the Health Care Navigator Nurse Coach Program, administered by American Health Data Institute (AHDI).
The AHDI Health Care Navigator Program is anchored by Health Care Navigator Wellness Coaches (registered nurses) who will review the status of your condition and help design a personalized management plan. The goal is to assure that you and your dependents get the full benefit of recommended testing and treatment and prevent the progression of a chronic condition and its associated complications.
How does the Disease Management Program work? |
Step 1: If you have a qualifying chronic condition you may receive an introductory letter inviting you to partner with a Health Care Navigator Wellness Coach. This letter will provide information about your condition(s) and the standards of minimum medical services that are recommended for the condition. It will also offer a list of telephone numbers, websites and other sources of related education materials. |
Step 2: Following the introductory letter, you can contact a Wellness Coach or they will reach out to you. The Coach will assess your current health status and your needs for additional personalized service. If appropriate, follow-up phone calls will be scheduled to assist you in exploring possible choices for self-managing your condition(s). |
Step 3: You and your Wellness Coach will discuss your needs and develop a personalized action plan together. |
